Clinical Review of Early Gastric Cancer with Lymph Node Metastasis



Abstract
In our country, the incidence and mortality rate of gastric cancer are most prevalent among all malignancies and usually show poor prognosis. However, the prognosis is excellent in early gastric cancer(EGC) compared to that of advanced gastric
cancer.
Especially, the prognosis of node-positive EGC is in improving state by more extended lymphadenectomy and offers nearly same curability as node-negative EGC.
We analyzed the clinical aspects of 33 cases of node-positive EGC compared to the node-negative EGC those were operated from Jan. 1988 to Dec. 1993 at the Department of General Sur gery, Hanyang University Hospital.
@ES The results are as follows:
@EN 1) the incidence of node-positive(10.8%) and 26 cases among 93 submucosal cancers were node-positive(28.0%).
3) Male to female ratio in node-positive group was 2.1 and the most prevalent age group was the 5th decade(44.4%).
4) The most common macroscopic type was IIc.
5) In submucosal cancers, elevated type showed higher incidence of node-positive ratio(52.0%) than non-elevated type(22.4%)(P=0.017).
6) The incidence of lymphatic invasion was higher in node-positive group(51.5%) than in node-negative group(20.8%)(P<0.001).
7) In node-positive group, the average number of lymph node from the resected specimen was 35.73 and average number of metastatic nodes was 2.78.
8) The preoperative serum level of carcinoembryonic antigen(CEA) was 3.2§¶/l in node-positive group, 2.9§¶/l in node-negative group and the incidence of abnormal level was 21.2% in node-positive group, 8.2% in node-negative group(P=0.047).
9) During the follow up periods, 3 cases were recurred(2 cases with regional lymph node metastasis and 1 case with ovarian metatasis).
10) Overall five year sarvival rate of all cases of EGC was 92.8%(node-positive group was 92.1% and node-negative group was 93.1%).
